Riverside Physician Network Case Management Program has been developed to provide assistance to members and their families who are experiencing
multiple or complex medical problems. We can assist you by coordinating the information, alternatives, and resources necessary to support the
member's treatment and recovery.
Case Management Can Help You:
Each nurse provides personalized service to you, your family, and your doctors to help coordinate your care. Your Case Manager will seek to help
plan for the complex array of services your special case requires and will make efforts to see that your care is administered in an efficient
and effective manner. If you have difficulty obtaining services, your Case Manager can assist in identifying options and ensuring you get all
the help you are entitled to receive.
Referrals to the Case Management Program:
You can request Case Management services yourself or you may be referred by your physician or other provider, a family member, or caregiver.
Your participation is voluntary. You can contact the Patient Advocate at 951-788-9800 or
866-535-0250 to ask for Case Management Services. Deaf, hard of hearing, or speech impaired members can access TDD/TTY services by calling 711.
